Registration Instructions:

1. After contacting our office, you will receive an email with your unique username and temporary password. To contact our office, click here.

2. Log on to the portal with the provided username and password.

3. Complete the initial login to authenticate your identity.

4. Click on the Questionnaire section

5. Complete all of the questionnaires listed prior to your visit.
